Transfers Originating Inside the United States:
- UCLA Rare Brain Disease Tissue Bank Institutional Review Board (IRB) Approval Letter
This is an approval letter from the UCLA IRB to accept samples in the tissue bank. This form is often required by the donating hospital to demonstrate regulatory approval for the tissue bank.
- Material Transfer Agreement (MTA) Draft
MTAs are agreements between hospitals that allow for the transfer of biological material between the two entities. Some hospitals and research institutions have their own templates and processes for generating MTAs. The RBDTB is flexible and we can work with any process necessary for the transfer. A template is provided here for convenience.

This form is a declaration required to import biological material into the United States. This form replaces the USDA Statement, and a draft is provided here that can be modified per transfer as necessary.
