Department Overview
The Department of Care Coordination and Clinical Social Work was established in 2004 and serves all patients within the hospital system as well as pediatric, oncology, and transplant patients in UCLA Westwood Clinics.
The department upholds the CORE Values of UCLA which are Compassion, Respect, Excellence, Discovery, Integrity and Teamwork.
Within the department, Administrative Staff, Clinical Case Managers, Clinical Social Workers, and Nurse Practitioners work together to support patients and their families. Our practitioners provide this support in the Emergency Departments, Hospital Inpatients Units, and as patients discharge or transition to post acute facilities.
Initial and ongoing assessments of patients occurs through this department. Coordination of care occurs while patients are hospitalized and during transition from the hospital setting to home or post-acute facilities.
The department provides information regarding community resources to assure continuity of care and provides timely transfer of medical information. We are committed to ongoing quality improvement initiatives that create efficiency, ensure optimal patient outcomes, and seamless transitions of care.
