Contact your primary care office to schedule an extensivist consultation appointment or call 310-206-6232(Link opens phone app) and request an appointment with one of our extensivist physicians. This consultation is available for those that have UCLA primary care and specialty doctors.

Please bring a list of all of your doctors and specialists you are currently seeing.

Based on the consultation, we may have you meet with one of our MyMeds pharmacists to help you better evaluate your medication regimen.

Based on the consultation, we may make recommendations to connect you with additional UCLA programs and resources.
