It's now easier than ever to refer your patients to services at UCLA Health. No matter where you're located, our physician referral team will provide you with the personalized help you and your patients require, with just a phone call, fax or e-mail.
CALL: 310-825-2631. We give personal attention to every call we receive. Our physician referral team is available to help Monday to Friday, from 7 am to 7 pm.

FAX: Fax your patient referral to 310-301-5391. We will call your patient to coordinate the referral. For any questions concerning your faxed referral, please call 310-825-2631.

Our partnership with health professionals in the community is key to our success at UCLA Health. It is our goal to assist you with referring a patient to UCLA and ensure they receive the care they need. Referral team members are available to assist referring physicians and case managers access services at UCLA Health by calling 310-825-2631. Or, send an email to [email protected].
Quick turn around time.
When doctors fax a patient referral to (310) 301-5391, the physician referral representatives will call your patient the same day or by the following morning to coordinate a referral.

Hospital-to-Hospital Transfer
The UCLA Transfer Center (310-825-0909) may be able to further assist in facilitating transfers to UCLA Health hospitals.
Our goal is to help patients and surrounding referring facilities when transfer requests are initiated. To do so, we are asking to have all essential patient information collected and readily available when a transfer request is initiated. Please download our transfer packet below and be prepared to answer the questions and fax information from this packet.
Once all this information is gathered, be prepared to verbally answer the questions as previously directed. Call the UCLA Patient Transfer Center at 310-825-0909. Then select the best fitting option for your patient referral from the list below.
- Option 1: MD, life threatening patient referrals, or ER Referrals.
- Option 2: Transplant patient referrals.
- Option 3: RN or CM urgent or non-emergent transfers.
