Registration is limited. An Early Bird registration fee of $1000 for faculty and $500 for fellows, residents, and APPs. The cost increases to $1250 and $625, respectively, after August 30th. Registration is non-refundable after September 1. A deposit of $100 is required to hold your spot in the course which is returned to attendees only after participation.
