Urology Resident Verifications
To start the process for a current or previous Urology resident verification, please send us an email. Be aware that there may be fees associated with specific requests. Our team will inform you of any fees once your request has been submitted.

In your request, please provide the following information:
- Applicant's full name
- Dates they attended
- Specialty
- Whether they are a resident or a fellow
Potential Fees:
- $50 for a standard form
- $75 for a form filled out by the Program Director
- $100 for a notarized form
