Not all insurance companies will cover the cost of visits with a dietitian. Follow the steps below to check if your session will be covered and what your copay amount will be:
- Call telephone number provided on the back of your insurance card
- Provide insurance company with CPT (billing code) and Tax ID number
- CPT - 97802 (new patient) or 97803 (return patient)
- Tax ID number - 954373071
HMO
- Referring physician’s office will need to request authorization for HMO patients
- Authorization needs to be approved before the day of the session
Medicare
- Visits with a GI dietitian will NOT be covered
- Only covers for diagnoses of obesity, diabetes and kidney disease as the primary diagnosis, even if you have Part B or secondary insurance
- Self-pay options are available (see below)
- Patient must sign an Advance Beneficiary Notice (ABN) of non-coverage before the session
- If the patient elects to have insurance billed and it is not covered, patient will be 100% responsible for the cost determined by the insurance company
Self-Pay
- Self-pay (fee for service) rate: New Patient: $200, Return Patient: $100
- Patient must sign a self-pay form of non-coverage before the session
