Step 1: Download and fill out our customized questionnaire (PDF) to provide us with a comprehensive summary of your assessment of your goals.

Step 2: Meet with our team nurse, who will take your vital signs and perform a 12-lead electrocardiogram (if indicated by your answers to our questionnaire).

Step 3: Meet with a sports cardiologist from our team. In addition to obtaining a complete medical history and performing a full physical exam, our cardiologist will spend time to fully understand your goals and help determine our role in helping you to achieve those goals.

Step 4: Further studies may be ordered by your sports cardiologist, depending on your specific symptoms or condition. The indication and utility for these tests will be described to you in detail, and if desired, the tests can be scheduled.
