I am a new patient to your practice, how do I select a doctor?

You may go to our website and find a doctor. Here we have information to help you select one of our outstanding physicians.

How do I contact the office?

Please call us directly at our office number 310-794-7274.  Please listen carefully to select the option that best suits your need.  During business hours (8 am to 4:45 pm ) there is an option (#1) to speak immediately with one of our receptionists.

What insurance plans do you accept, and will my visit and all related services be covered by my insurance?

We accept Aetna, Blue Cross, Blue Shield, Pacific Care, Medicare, and numerous commercial insurances. You need to check with your insurance carrier regarding specific coverage questions. Due to the vast array of plans available, your carrier is best equipped to assist you with this question

What if I have an HMO?

Many HMOs will authorize referrals to UCLA. You should obtain an authorization from your HMO primary care provider before you are seen at UCLA.

How long will I have to wait for my first appointment?

Most often appointments can be scheduled within 2 weeks.

How do I obtain medical records?

Please complete the medical records release form and fax to us at 310-206-0043 or bring to office. The Privacy Management office has 14 days to complete your request for copies of your medical records.

I have been referred to your department, What should I bring to my first visit?

Please bring any copies of your medical records, insurance card, list of current medications and consider making a list of questions you might have for your doctor.
