Learn about our methods for consenting

We make it easy, safe and free to complete the Universal Consent and allow researchers to use your leftover biological samples. It is a one-time process that takes under 10 minutes and involves watching a short video.

If you are over the age of 18 years, you can complete the consent online, during a medical appointment or inpatient stay at certain UCLA Health locations. We may send you reminders, too.

If you are the parent/legal guardian of a pediatric patient, you and the patient may only complete the Universal Consent in-person at certain UCLA Health locations. To view a list of participating pediatric locations, click here.

Complete online at home or on the go (Adult patients, only)

MyChart patient portal at myUCLAhealth.

If you have an account set up, we have a few options for you to access and complete your consent at myUCLAhealth.

  • The form is listed under the “Questionnaires” tab for both mobile and desktop formats. It’s available 24/7.
  • If you have an upcoming appointment, you may receive a MyChart message notifying you that you can complete the Universal Consent by clicking on the link in the message. 
  • When you log into your account, you may also see an announcement on your homepage where you can click “Complete Today” to be directed to the Universal Consent.
  • Patients staying at one of our inpatient units can use the bedside iPads available in each room. Click on side menu option “a few questions” and, if you have not completed it, you will see the Universal Consent listed and you will be able to click on it to access the Universal Consent. 

    If you need to, you can pause while completing the form and return to it later within 7 days of starting the form.

Appointment Reminder Text Messages

  • If you have chosen to receive text message reminders of your UCLA Health appointments, you may see a link to the Universal Consent at the bottom of the message. You can click on this link up to two weeks after you received the message to access and complete the Universal Consent. You will need to enter your cell phone number for verification.

  • To complete the consent online at any time in English, Spanish, Farsi, Mandarin, Korean or Arabic, please click here
  • You may also scan the QR code displayed on this page with your smart device, phone or tablet, camera and tap the link that appears. Select your language preference and enter your medical record number to follow the prompts and complete the Universal Consent for Biological Samples. 

Complete on site at participating locations (Adult and pediatric patients)

A number of outpatient participating locations offer the opportunity to complete your consent while you wait. Some of these locations may feature trained volunteers or staff who can help you navigate the consent process, answer questions, and troubleshoot issues.

Scan a QR code (Adult patients, only)

  • Front desk staff will provide a flyer about the program that contains a QR code. The camera on your mobile device scans the code and takes you to the online consent. To begin, you will need your medical record number, which the front desk staff will provide. 

Use an iPad

  • Front desk staff can also provide an iPad with the Universal Consent web app. To begin, you will need your medical record number, which the front desk staff will provide. Staff disinfect these devices after each use. 

    Unfortunately, there is no way to save your progress if your appointment begins before you finish, however, you may start again after your appointment.

    Pediatric patients and their parents/legal guardians may only complete the Universal Consent on an iPad in-person at certain UCLA Health participating locations.
