Billing

The UCLA Comprehensive Health Program (CHP) services are available to companies and individuals. We operate through company and private billing to provide our services. Companies can arrange to be billed directly for the CHP services provided to their executives. Please contact your Human Resources department or call us directly to see if you are eligible for a UCLA Executive Physical. If you are a Human Resources representative, we welcome your call to discuss establishing a contract with our program.

We offer UCLA Executive Physical packages for executives who wish to participate in our program without a company contract. Please call us to schedule your visit and discuss billing.
