Medical Licensure Policy
California Medical Board and Osteopathic Medical Board will no longer accept paper-based application forms. Applications should be submitted electronically through BreEZe.
Apply at least 6 months in advance for your medical license.
A trainee must obtain a Postgraduate Training License (PTL) within 180 days after enrollment in a Board-approved postgraduate program.
In the event that licensure is not obtained, the trainee will be immediately suspended from the program. No credit for residency training will be given for the time under suspension.
Reinstatement to the program will require adequate documentation of licensure.
Postgraduate Training License (PTL) requirement:
- Graduated from board-approved medical school
- Passed USMLE Steps 1 & 2CK or part 1 of LMCC/Canadian exams
- If graduated from an international medical school (other than Canada) must have a valid ECFMG certificate
Physician’s and Surgeon’s License (P&S) requirement:
Attended and graduated from a U.S., Canadian, or an International Medical School approved by the Board pursuant to Business and Professions Code (BPC) section 2084;
- Received credit for either 12 (United States and Canadian medical school graduates) or 24 (international medical school graduates) months of board-approved postgraduate training (BPC section 2096); and
- Passed all required examinations (BPC section 2170).

Drug Enforcement Agency (DEA) Certification Policy
Trainees are expected to obtain DEA certification after obtaining Postgraduate Training License (PTL). As a UCLA Trainee, you are eligible for a fee exemption for your DEA registration. Please note, an exempt fee DEA is only valid at the exempting institution and any affiliate hospital rotations within the scope of your training. An exempt fee DEA is not valid for moonlighting purposes.

When you receive your DEA certificate, please provide a copy to the surgery education staff. We will start your e-prescribing process.
