Referring patients to UCLA Urology is easy.

No matter where you're located, our UCLA Health referral team will provide you with the personalized help you and your patients need. We are just a phone call or e-mail away!

CALL: 310-825-2631(Link opens phone app). We give personal attention to every call we receive. Our physician referral team is available to help Monday to Friday, from 7 am to 7 pm.

EMAIL: access@mednet.ucla.edu. A member of our physician referral team will respond.

FAX: Fax your patient referral to 310-301-5391(Link opens phone app). We will call your patient to coordinate the referral. For any questions concerning your faxed referral, please call 310-825-2631(Link opens phone app).
