Evaluation of New Models of Coordinated Care Among Medicaid Enrollees with Diabetes and Prediabetes

University of California, Los Angeles and UnitedHealthcare (UHC) are partnering to evaluate two interventions designed to better manage care for UHC Medicaid members. Accountable Care Communities is an UHC-led initiative that provides community practices with training, real-time clinical dashboards, and shared savings to improve care delivery for high-risk members. Health Homes are a model of “whole-person” care for patients with chronic conditions authorized by the Affordable Care Act. The evaluation will utilize longitudinal, quasi-experimental approaches that rely on contemporaneous comparison groups to estimate the impact of these interventions on clinical, utilization, and cost outcomes among persons with diabetes and pre-diabetes.
